Please remember to bring your child’s insurance card to each visit. Your insurer requires us to collect all co-payments and deductibles at the time of service.

If you are not an established PM Kidz patient, you should contact us before your appointment to verify that your insurance is accepted for acute care visits.

Sliding Fee Discount Program

At PM Kidz, no patient will be denied medical services due to an inability to pay. We offer a sliding fee discount program for those who qualify. To determine your eligibility, we require verification of identification and income, which must be updated annually to continue your participation in the program.
